Plenty of Famicom games went through radical changes in their transition to the West. Even Nintendo’s own titles got major makeovers from time to time. The North American version of Super Mario Bros. 2 is one of the most notable examples, but plenty of other first party titles went through numerous revisions and tweaks before localization.

One example of localization differences from Nintendo is the original Legend of Zelda. While the two games might look quite similar at first glance, there’s actually a considerable amount of disparity between the two versions. These changes go far beyond the usual text tweaks and difficulty adjustments.
